Your interdisciplinary group
The hospice interdisciplinary group—often called the IDG—reviews each patient’s needs together and updates the plan of care as conditions, symptoms, and family needs change.
Confirms hospice eligibility, helps guide the comfort-focused medical plan, and collaborates with the patient’s attending clinician and hospice team.
Coordinates the plan of care, assesses symptoms, teaches patients and caregivers, manages clinical needs, and communicates changes to the team.
Provides emotional support, helps families navigate difficult decisions, connects them with resources, and assists with practical care concerns.
Offers patient-led spiritual and emotional support that respects each person’s faith, culture, values, and preference—including the choice to decline.
Makes nursing visits under RN direction, monitors symptoms, reinforces teaching, provides ordered care, and reports changes promptly.
Supports bathing, grooming, dressing, positioning, and other personal-care needs while observing comfort and reporting changes to the nurse.
Offers grief education, supportive outreach, and bereavement resources to loved ones before and after a death.
The team’s role is to listen, explain options, and coordinate care around what matters most. Visit types and frequency are based on the individualized plan of care.

A simple conversation can help you understand whether hospice may be appropriate now. There is no pressure to enroll.

The patient chooses care focused on comfort and quality of life for the terminal illness rather than treatment intended to cure it.

Understanding Medicare hospice coverage
Medicare Part A covers hospice care through a Medicare-approved hospice provider when all eligibility requirements are met.
View official Medicare hospice information →A patient generally qualifies when the patient has Medicare Part A; the hospice physician and attending physician, if applicable, certify a life expectancy of six months or less if the illness follows its expected course; the patient chooses comfort-focused care instead of treatment intended to cure the terminal illness and related conditions; and the patient signs a Hospice Election Statement.
The hospice benefit generally covers care included in the individualized plan of care, including services, medications, equipment, and supplies related to the terminal illness and related conditions. Care for the terminal illness must be provided or arranged by the selected hospice team.
Hospice may be provided in a private home, assisted-living community, nursing facility, or another place the patient calls home. Medicare may also cover hospice-arranged short-term inpatient or respite care when requirements are met.
Patients and families should contact the hospice team before obtaining emergency-room care, hospitalization, ambulance transportation, medications, or other services related to the terminal illness. Services not arranged by the hospice may become the patient’s financial responsibility.
A patient may continue seeing a regular physician or nurse practitioner when that clinician is selected as the attending medical professional.
Medicare provides two initial 90-day hospice benefit periods, followed by an unlimited number of 60-day periods. Hospice may continue beyond six months when the patient remains eligible and the hospice physician completes the required recertification.
Patients may request a written explanation of items, medications, or services the hospice determines are unrelated to the terminal illness and therefore not covered by the hospice benefit.
Coverage depends on Medicare requirements, the patient’s eligibility, and the individualized plan of care. Charity Hospice Care will explain anticipated coverage and potential costs before services begin.
